Intuitive Canine Release of Liability Terms

  

I hereby agree to be responsible for the actions and conduct of my pet and to release Intuitive Canine and City Dog Market from any claims and demands that may arise during participation in training, walks, board & train, play, or leisure activities.  

By agreeing to this form, I confirm that my pet(s) are up to date on their vaccines. 

 

I agree to release Intuitive Canine, its employees, and myself from any liability that may arise or result from any incidents or accidents involving my pet or pets, to the maximum extent allowed by law.

 

Furthermore, all products and information, including but not limited to: training, worksheets, information, brands, ideas, and courses, are the proprietary property of Intuitive Canine. The information given is copyrighted and cannot be copied, shared, or distributed without consent. 

 

I agree that payment is due upon receipt of services being rendered, and payment is non-refundable. There is no option to reschedule missed classes.
